Context and use

Clinical score = morning stiffness >45 min (2) + hip pain/limited ROM (1) + absent or normal RF or ACPA (2) + absence of peripheral joint pain (1)

Without ultrasound: classify at >=4 points. With ultrasound: add 1 point for >=1 shoulder plus >=1 hip with qualifying findings and 1 point for both shoulders, with a >=5 threshold.

Apply only after confirming the entry conditions of age >=50 years, bilateral shoulder aching, and abnormal ESR and/or CRP.
