4AT = alertness (0 or 4) + AMT4 (0/1/2) + attention with months backwards (0/1/2) + acute or fluctuating change (0 or 4)

Total score 0-12. Official interpretation: 0 unlikely delirium or moderate-severe cognitive impairment; 1-3 possible cognitive impairment; >=4 possible delirium.
