Context and use

APPM = prior episodes (0-4) + cholelithiasis (2) - recent abdominal surgery (2) + epigastric pain (2) + worsening pain (1) + pain duration >=5 days (-1) + pain severity (mild 0, moderate 2, severe 3) + lipase category (0-2).

Operational bands: <=2 low risk, 3-5 medium risk, 6-7 high risk, and >=8 very high risk. The >=6 cutoff had the best overall accuracy in prospective validation.
