Surgery type: CABG only 0, valve only 1, CABG + valve 2, other cardiac surgery 2.
Preoperative creatinine <1.2 mg/dL = 0 points; 1.2 to <2.1 mg/dL = 2 points; >=2.1 mg/dL = 5 points.
