Context and use
Total score = age + sex + gestational diabetes + family history + hypertension + inactivity + body-size points
Age: <40 = 0, 40-49 = 1, 50-59 = 2, >=60 = 3. Man adds 1 point; gestational diabetes adds 1 point only for woman.
Body-size points (0-3) are determined from the ADA/CDC height row in inches and weight in pounds. A positive screen is total score >=5.
