ADAPT Protocol for Cardiac Event Risk

CardiologyChest painEmergency medicineBeta toolThis tool is in beta. It has undergone internal validation but still requires additional testing. Always confirm the data entered and the results obtained before clinical use. It should not be used as the sole basis for clinical decisions.
Only TIMI score 0 can satisfy the original ADAPT low-risk rule.
New ischemic ECG changes presentAny new ischemic ECG change makes the presentation not low risk.
0-hour troponin negative by the validated local assay cutoffEnter whether the initial troponin was interpreted as negative by the local assay.
2-hour troponin negative by the validated local assay cutoffADAPT requires an explicit 2-hour troponin interpretation before low risk can be assigned.

Clinical noticeEducational protocol-support tool only. Use it only inside a validated local chest-pain workflow and alongside full clinical assessment.

Context and use

ADAPT low risk = (TIMI = 0) and (ECG without new ischemia) and (0-hour troponin negative) and (2-hour troponin negative)

If any criterion fails, the presentation remains not low risk / ADP-positive.

This calculation follows the original contemporary-troponin ADAPT workflow rather than later high-sensitivity troponin variants.
