AKIN Classification for Acute Kidney Injury (AKI)
Use the same unit for baseline and current creatinine when that pair is entered.
Enter baseline creatinine only together with current creatinine and the 48-hour confirmation field.
Provide a current creatinine value greater than 0 in the same unit as baseline.
Enter urine output already normalized to mL/kg/h.
mL/kg/h
Required when a urine output rate is entered. AKIN thresholds use durations of >6 h, >12 h, and >=24 h.
h
Anuria for 12 hours or longer contributes AKIN stage 3.
h
Clinical noticeEducational support for historical acute kidney injury classification only. Use current guideline-based assessment and full clinical context.
Context and use
Final AKIN stage = max(creatinine stage, urine-output stage, RRT stage).
Creatinine: stage 3 if ratio >3.0 or current creatinine >=4.0 mg/dL with rise >=0.5 mg/dL (>=354 umol/L with rise >=44); stage 2 if ratio >2.0; stage 1 if rise >=0.3 mg/dL (26.4 umol/L) or ratio >=1.5.
Urine output: stage 3 if anuria >=12 h or <0.3 mL/kg/h for >=24 h; stage 2 if <0.5 mL/kg/h for >12 h; stage 1 if <0.5 mL/kg/h for >6 h.
