Context and use

APACHE II = APS + age_points + chronic_health_points, where APS is the sum of point bands for temperature + mean arterial pressure + heart rate + respiratory rate + oxygenation + acid-base + sodium + potassium + creatinine + hematocrit + white blood cells + neurologic score. Oxygenation uses A-aDO2 when FiO2 >= 0.50, otherwise PaO2. Acid-base uses arterial pH, or serum bicarbonate only if no arterial blood gases are available. Neurologic score = 15 - GCS. Creatinine points are doubled when acute_renal_failure is marked true. Age points: <45 = 0, 45-54 = 2, 55-64 = 3, 65-74 = 5, >=75 = 6. Chronic health points: 0 if no qualifying chronic health history; 2 if elective_postoperative with qualifying chronic health history; 5 if nonoperative or emergency_postoperative with qualifying chronic health history.
