Context and use

Compute ln(age), ln(total cholesterol), ln(HDL-C), and ln(systolic blood pressure), then apply the official sex- and race-specific ACC/AHA Pooled Cohort Equation coefficients, mean terms, and baseline survival constants from the public ACC implementation. Ten-year risk = 1 - baseline_survival ^ exp(linear_predictor - mean_term).
